Getting a Wegovy Prescription Without Guesswork

Wegovy is semaglutide, a GLP-1 receptor agonist approved for chronic weight management in adults with a BMI of 30 or higher, or 27 or higher with at least one weight-related condition such as type 2 diabetes or high blood pressure. The first practical step is not a website but a clinician willing to document that BMI and any comorbidity in your chart. Primary care offices, endocrinology practices, and many obesity medicine clinics handle this; some telehealth platforms do too, but they still need recent vitals and labs.

The prescription itself is straightforward once the criteria are met. The friction comes afterward. Pharmacies in the U.S. have spent the last few years cycling through supply gaps for the starter doses, so a prescriber who knows the local landscape matters. A clinic in a mid-sized market like Bradenton, Florida, for example, may steer patients toward a specific pharmacy that reliably stocks 0.25 mg pens. Ask about that before you leave the appointment.

Beware of any site offering to sell you Wegovy online without a prescription. Novo Nordisk does not sell direct to consumers, and the compounded semaglutide that filled the gap during shortages is a separate product with its own regulatory status. If a seller asks only for a credit card and a questionnaire, walk away. A legitimate online path routes through a licensed prescriber in your state and a licensed pharmacy that ships to your address.

What Medicare Actually Covers

Here is the answer most people over 65 need: original Medicare generally does not cover Wegovy or other GLP-1 drugs when they are prescribed for weight loss alone. Part D plans are barred from covering drugs used for weight loss or weight gain under the Social Security Act. That rule has held for two decades despite repeated legislative attempts to change it.

The picture shifts when the same drug is prescribed for a covered indication. Wegovy carries an FDA approval for reducing cardiovascular risk in adults with established heart disease and overweight or obesity, and Part D plans can cover it for that purpose. Ozempic, the same molecule at a different dose, is covered for type 2 diabetes. If you have both diabetes and obesity, your prescriber should code the diagnosis carefully — the indication on the claim determines whether the plan pays.

Medicare Advantage plans sometimes go further than original Medicare because they can offer supplemental benefits, but coverage varies by plan and county. Call the plan’s member services line and ask specifically whether GLP-1 drugs are on the formulary and what prior authorization requires. Get the answer in writing if you can. Without coverage, list prices for Wegovy run around $1,300 to $1,400 per month, though Novo Nordisk’s direct-to-consumer pharmacy now sells it for roughly $499 monthly for cash-paying patients.

Semaglutide Timeline and the Generic Question

Patients often ask how long semaglutide takes to work. The honest answer is that the scale moves slowly by design. The dosing schedule ramps from 0.25 mg to 2.4 mg over about four months to limit nausea and other gastrointestinal side effects. Meaningful weight loss typically shows up by week eight to twelve, and the trial averages — roughly 15 percent of body weight over 68 weeks — reflect people who stayed on the full dose. Drop out early and the numbers look different.

Generic semaglutide is not available in the U.S. yet. The core patent runs into the early 2030s, though litigation and challenges in other countries have chipped at the edges. For now, any product labeled “generic semaglutide” sold to American consumers is either compounded, imported illegally, or mislabeled. Compounded versions were permitted during official shortages but the FDA has been winding that flexibility down as supply recovers.

Cost planning matters as much as the drug itself. Check whether your employer plan or Marketplace plan lists Wegovy as a preferred brand, ask about manufacturer savings cards (generally not usable with Medicare or Medicaid), and confirm the prior authorization paperwork is filed before your first fill. A denial is common on the first pass and usually reversible with a letter from the prescriber documenting BMI history and any failed lifestyle programs.

Related Conditions That Shape the Decision

Weight-loss drugs rarely exist in isolation. Many people considering Wegovy also manage type 2 diabetes, which means the conversation overlaps with metformin, A1C targets, and lab work. If you have not had a diabetes type 2 test recently, an A1C and a fasting glucose are the standard starting pair; some clinicians add an oral glucose tolerance test or a random glucose with symptoms. Knowing your numbers changes which drug your insurance will approve and which diagnosis code goes on the claim.

Comorbidities drive the economics. A patient with diabetes, sleep apnea, and hypertension has a stronger case for coverage than someone with obesity alone, and the clinical rationale is genuine — untreated diabetes comorbidities cost far more over a decade than the drug does. Bring your full problem list to the appointment rather than framing the visit as a weight question only.

Finally, set expectations about duration. These are chronic medications for most people. Stopping after six months usually means regaining most of the lost weight within a year. Budget for the long term, confirm coverage annually during open enrollment, and revisit the plan every fall before premiums and formularies reset.
